چکیده

Achalasia is characterized by loss of coordinated esophageal peristalsis and failure of the lower esophageal sphincter (LES) to completely relax. High resolution manometry distinguishes three subtypes [1, 2]. The Eckardt score evaluates clinical symptoms, but not quality of life [3]. The treatment aim is to decrease the LES pressure and the integrated relaxation pressure (IRP), in order to improve symptoms [4]. Medical and endoscopic therapeutic options have limited efficacy [5]. Laparoscopic Heller myotomy is considered the gold standard of treatment [6], but peroral endoscopic myotomy (POEM) has gained popularity because of its excellent safety and efficacy [7–12]. Success is defined as Eckardt score <3, IRP <15mmHg, and >80% emptying of contrast at 5 minutes in TBE. Long-term quality of life after peroral endoscopic myotomy remains compromised in patients with achalasia type III.

Background and study aims Peroral endoscopic myotomy (POEM) is an excellent endoscopic treatment for achalasia. Clinical and manometric parameters are used for evaluation and follow-up. However, clinical success does not guarantee high quality of life (QoL) scores, generating doubts about their direct relationship. We aimed to evaluate QoL scores before and after POEM at medium and long term, t...

Peroral Endoscopic Myotomy for the Treatment of Achalasia in a 10-Year-Old Male Patient

Peroral endoscopic myotomy (POEM) is a new endoscopic treatment for achalasia with very good short-term results in adults. Data about POEM in pediatric patients are missing. We present the case of a 10-year-old male patient with type I (classic) achalasia, successfully treated with POEM. The procedure was accomplished in a similar fashion to the technique used in adults. Short-term results were...

Successful peroral endoscopic myotomy performed in Endoscopy Department as a radical, long-term treatment for esophageal achalasia – the Greek experience

INTRODUCTION Peroral endoscopic myotomy (POEM) has been considered as a minimal-invasive, innovative technique for long-term treatment of all types of esophageal achalasia and other esophageal motility disorders. PATIENTS AND METHODS We report on 20 consecutive Greek patients with manometrically proved esophageal achalasia (14 patients with type I, 4 with type II, 2 with type III, and 4 with ...

Myotomy of Distal Esophagus Influences Proximal Esophageal Contraction and Upper Esophageal Sphincter Relaxation in Patients with Achalasia After Peroral Endoscopic Myotomy

BACKGROUND/AIMS The motility change after peroral endoscopic myotomy (POEM) in achalasia is currently focused on lower esophageal sphincter (LES). This study aims to investigate the correlation of motility response between distal and proximal esophagus after POEM. METHODS A total of 32 achalasia patients who received POEM and high-resolution manometry (HRM) were included for analysis. Eckardt...

The Outcomes and Quality of Life of Patients with Achalasia after Peroral Endoscopic Myotomy in the Short-Term.

PURPOSE To assess the short-term efficacy and quality of life (QOL) of patients with achalasia after peroral endoscopic myotomy (POEM). METHODS Thirty-five achalasia patients underwent POEM from May 2013 to December 2013. The data on clinical evaluation and QOL before therapy, at 1 month and 6 months postoperation were collected and analyzed. RESULTS All the thirty-five patients underwent P...
